That GP may handle most of your care, such as:
- pre-bookable routine appointments
- managing long-term conditions
- referrals to specialists
Please note that by nominating your preferred GP you will not always be able to see your preferred GP on demand for same day appointments.
You can use this service if you:
- are registered at the surgery
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
- your preferred GP
