To help us keep your medical records accurate and ensure we can communicate with you effectively, we are asking all patients to check and update their contact details.
If a family member or someone else regularly contacts the surgery or speaks to us on your behalf, please provide the following information:
Patient’s full name
Name of the family member or representative
Family member’s telephone number
Patient’s signature confirming permission for this person to speak on their behalf
Please also let us know if any of your own contact details have changed, including your address, telephone number, or email address.
Keeping your records up to date helps us contact you when needed and ensures we have the correct information to provide you with the best possible care. If you need to update your details, please speak to a member of our reception team or complete an update form at the surgery.
